How to File a Liability Claim Form Against Cathedral City

To file a claim against Cathedral City please use the following form:

In order for the City to process your claim in a timely manner, please completely fill out the form and submit it to the City Clerk's Office at the address below.  An original form with a wet signature must be submitted.

You may attach documents such as incident reports, invoices and photos along with your completed claim form.  Forms submitted without an original signature or incomplete may result in delays in processing the claim. Be sure your claim is against the City of Cathedral City, California, and not another public entity as this may impact your ability to file a claim with the appropriate agency due to filing deadlines.  If you have any questions please contact the City Clerk's Office at cityclerk@cathedralcity.gov or call 760 770-0322.

  1. Claims for death, injury to person or to personal property must be filed no later than 6 months after the occurrence.
  2. Claims for damages to real property must be filed no later than one year after the occurrence. (See Government Code §911.2)
  3. Be sure your claim is against the City of Cathedral City, not another public entity.
  4. Claimant is advised to consult a private attorney if legal advice is desired. No employee of the City may give legal advice to any claimant relating to private claims.
